Our mission is to help employees return to work quickly, safely, and at peak function while minimizing claim duration and maximizing recovery potential. We are not contracted with any PPO or WC network and do not accept repriced rates. Services rendered will be billed at our WC fee schedule unless otherwise agreed in writing. Please have your adjuster email us at records@connorzieglermd.com to review rates and sign agreement form.
If you have a work injury and going through worker’s compensation insurance, please inform our staff immediately.
Contact your adjuster and request a prior authorization for a new patient consultation. Dr. Ziegler’s National Provider Identifier (NPI) is 1174885883.
After you have obtained an authorization, you must provide our staff with the following information:
- Your claim number.
- The name of your company’s worker’s compensation insurance.
- Your employer’s name, telephone number and email if available.
- Your adjuster’s name, telephone number and email.
To expedite processing, please include:
Employee Information
- Full name
- Date of birth
- Contact phone/email
Claim Information
- Claim number
- Date of injury
- Accepted/authorized body part(s)
- Adjuster name and direct phone/email
- Employer name and HR contact
Medical Information
- Any prior imaging (MRI, X-ray)
- PT or prior treatment notes
- Prior surgical history (if relevant)
This information ensures fast scheduling and accurate documentation.

An intake form and questionnaire will be emailed along with secure link to upload pertinent medical documents and imaging. If outside imaging was done, it is recommended to either upload the imaging from a disc prior to the appointment (link will be provided for upload) or for the injured patient to bring a disc of their imaging with them to their appointment.
